HIPAA & Protected Health Information Policy

This HIPAA & Protected Health Information Policy (“HIPAA Policy”) works in conjunction with our Privacy Policy, and contains information about our use and disclosure of protected health information (“PHI”) according to the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).  

Collection of PHI: 

We may collect various types of PHI from our customers about their patients, including identifiers (e.g., name, address), health information (e.g., treatment information), and other personal details. This information is collected to provide our products and services and improve our offerings.
 

Use, Retention, and Disposal: 

PHI is used solely for the purposes stated at the time of collection, such as providing our data analytics services to our customers. We retain PHI for as long as necessary to fulfill these purposes and comply with legal requirements. Secure disposal methods are employed to ensure information is destroyed when no longer needed.
 

Disclosure to Third Parties: 

We do not sell the PHI we collect from our customers.
 

Security for Privacy: 

We employ a range of administrative, physical, and technical safeguards to protect PHI from unauthorized access, use, or disclosure. These measures follow established industry best practices, and include encryption, access controls, and regular security audits.
 

Breach Notification: 

In the event of a breach of unsecured PHI, we will notify affected our customer without unreasonable delay and in accordance with applicable laws and regulations, to enable our customer to notify affected individuals. Notifications will include information about the breach, the affected information, and other information we are required to provide under HIPAA and other applicable data privacy laws.
